As a large number of people showing no symptoms being detected for COVID-19, the Union health ministry on Thursday revised the home isolation guidelines to include <keyword id="9211271" type="UNKNOWN" weightage="20" keywordseo="Asymptomatic-positive-patients">asymptomatic positive patients</keyword> in the list of mild or pre-symptomatic coronavirus infection cases. Patients under home isolation will stand discharged after 10 days of onset of symptoms and no <keyword id="6387615" type="UNKNOWN" weightage="20" keywordseo="fever">fever</keyword> for three days, the guidelines stated. "Thereafter, the patient will be advised to isolate at home and self-monitor their health for further seven days. There is no need for testing after the home isolation period is over," stated the 'Revised Guidelines for Home Isolation of Very mild/pre-symptomatic/asymptomatic COVID-19 cases'. According to the document, asymptomatic patients like the ones who are pre-symptomatic and have very mild symptoms can opt for home isolation if they have the requisite self-isolation facility at their residence so as to avoid contact with other family members. The guidelines come against India's rising COVID-19 tally to 6,04,641 on Thursday, with a single-day increase of 19,148 cases, just five days after it crossed the 5 lakh-mark. The death toll due to the disease rose to 17,834 with 434 new fatalities, according to <keyword id="4956438" type="COMPANY" weightage="20" keywordseo="health-ministry-data">health ministry data</keyword> updated at 8 am. According to the guidelines, a care giver should be available to provide care on 24x7 basis and communication link between the caregiver and hospital is a prerequisite for the entire duration of home isolation. Besides, the guidelines reiterated that the care giver and all close contacts of such cases should take hydroxychloroquine as a preventive medication according to the protocol and as prescribed by the treating medical officer. Besides, it calls for downloading the Arogya Setu mobile application and that it should remain active at all times (through bluetooth and wi-fi). Patients should monitor their health and regularly inform the health status to the district surveillance officer, who will facilitate further follow up by the surveillance teams. The patient also has to give an undertaking stating being diagnosed as a confirmed/suspect case of COVID-19, he/she hereby voluntarily undertake to maintain strict self-isolation at all times for the prescribed period, they said. Immediate medical attention must be sought if serious signs or symptoms, including difficulty in breathing, dip in oxygen saturation, persistent pain/pressure in the chest, mental confusion or inability to arouse, slurred speech/seizures, weakness or numbness in any limb or face and developing bluish discolorations of lips/face, the guidelines stated. The guidelines state that states and districts should monitor all such cases, and that the health status of those under home isolation should be monitored by the field staff/surveillance teams through personal visits along with a dedicated call centre to follow up on the patients on a daily basis. The clinical status of each case should be recorded by the field staff/call center (body temperature, pulse rate and oxygen saturation). The field staff will guide the patient on measuring these parameters and provide the instructions (for patients and their care givers), they said. Besides, details about patients under home isolation should also be updated on COVID-19 portal and facility app (with DSO as user). Mechanism to shift patient in case of violation or need for treatment has to be established and implemented. All family members and close contacts shall be monitored and tested as per protocol by the field staff. These discharge guidelines shall be strictly adhered to along with issuance of a fitness certificate by the field team, they added. The CBI has booked Venkata Krishna Reddy Gunupati, the chairman of <keyword id="9500295" type="COMPANY" weightage="20" keywordseo="the-gvk-group-of-companies">the GVK Group of Companies</keyword> and his son G V Sanjay Reddy, Managing Director of Mumbai International Airport Ltd among others for alleged irregularities to the tune of Rs 705 crore in running of the airport, officials said on Wednesday. Airport Authority of India Ltd had formed a Joint venture with GVK Airports Holdings Limited, promoted by <keyword id="6608657" type="UNKNOWN" weightage="20" keywordseo="GVK-Group">GVK group</keyword> under public private partnership firm Mumbai International Airport Ltd for upgradation and maintenance of Mumbai Airport. On April 4, 2006 AAI entered into an agreement with MIAL for modernisation, upkeep, operation and maintenance of Mumbai airport with MIAL. It is alleged that promoters of GVK group in MIAL in connivance with their executives and unidentified officials of AAI resorted to siphoning of funds using different ways, officials said. The agency has alleged that they siphoned funds showing execution of bogus work contracts to nine companies in 2017-18 causing a loss of Rs 310 crore, they said. The promoters of GVK group allegedly misused reserve funds of MIAL to the tune of Rs 395 crore to finance their group companies, the CBI has alleged. The agency has alleged that the group inflated expenditure figures of MIAL by showing payments to employees at their headquarters and group companies who were not involved in the running of MIAL causing a revenue loss to AAI, they said. The promoters allegedly under reported revenue earnings of MIAL by entering into contracts with related parties and meeting their personal and family expenses using MIAL funds, they said. It's also important to note that getting a vasectomy reversal doesn't ensure a successful pregnancy. If you're considering a vasectomy reversal in order to have a child, it's important to know the odds as well as risks associated with the procedure.<h2>What is a vasectomy reversal? </h2>When someone undergoes a vasectomy, their vas deferens - or the tubes that carry sperm from the testicle to the penis - are severed or cut off. Therefore, a <a href="https://health.clevelandclinic.org/vasectomy-reversal-5-facts-men-need-to-know-2/" target="_blank" rel="noopener">vasectomy reversal</a> is a surgical procedure that re-connects these tubes, thereby restoring fertility. However, a 2018 <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC6194271/#rmb212207-bib-0003" target="_blank" rel="noopener">review</a> found that while the average rate of patency, or the presence of sperm in the semen, after vasectomy reversal was 87%, the average rate of pregnancy was only 49%. "Even when a patient has a successful return of sperm to semen, they may not be able to achieve pregnancy," Yih says. For example, pregnancy success rates can be as high as <a href="https://www.arizona-urology.com/blog/what-is-the-success-rate-for-a-vasectomy-reversal" target="_blank" rel="noopener">76% if the reversal is done within 3 years of the vasectomy</a>, but drop to about <a href="https://www.arizona-urology.com/blog/what-is-the-success-rate-for-a-vasectomy-reversal#:~:text=Of%20those%20whose%20reversals%20occurred,reversals%20show%20an%20increased%20success." target="_blank" rel="noopener">30%</a> after 15 years or more, says <a href="https://wexnermedical.osu.edu/find-a-doctor/jessica-yih-md-114054" target="_blank" rel="noopener">Jessica Yih</a>, MD, a urologist with <a href="https://wexnermedical.osu.edu/" target="_blank" rel="noopener">The Ohio State University Wexner Medical Center.</a> That's because the longer you wait to get a reversal, the more likely you are to develop blockages that prevent the sperm from coming out of the vas deferens. What's the cost? Vasectomy reversals are usually not covered by insurance, Pineda says. The cost of the procedure can <a href="https://www.urologyhealth.org/urology-a-z/v/vasectomy-reversal" target="_blank" rel="noopener">range from $5,000 to $15,000</a> depending on where you live and the rates your doctor's medical practice charges. <h2>How do vasectomy reversals work? </h2>A vasectomy reversal is done under general anesthesia and takes about <a href="https://www.mayoclinic.org/tests-procedures/vasectomy-reversal/about/pac-20384537" target="_blank" rel="noopener">two to four hours</a>, Pineda says. There are two procedures to reverse a vasectomy: <ul><li><a href="https://www.urologyhealth.org/urology-a-z/v/vasectomy-reversal">Vasovasostomy</a>, which connects the two ends of the tube carrying sperm back together. </li><li><a href="https://www.urologyhealth.org/urology-a-z/v/vasectomy-reversal" target="_blank" rel="noopener">Vasoepididymostomy</a>, which is a more complex procedure where the vas deferens are connected to a much smaller tube in the duct behind the testis, called the epididymis. </li></ul>Doctors will decide which procedure to perform after making two incisions on each side of the scrotum and extracting fluid from the testicle side of the vas deferens, Yih says. If the fluid contains sperm, they will perform a vasovasostomy. If it doesn't, then they will perform a vasoepididymostomy. Follow-up protocol after a reversal differs slightly among urologists, Pineda says, but in general, your doctor will test your semen every <a href="https://www.urologyhealth.org/urology-a-z/v/vasectomy-reversal" target="_blank" rel="noopener">2 to 3 months</a> to determine sperm count. And patience is key here, since it may take <a href="https://www.urologyhealth.org/urology-a-z/v/vasectomy-reversal" target="_blank" rel="noopener">4 to 12 months</a> for your partner to become pregnant after a vasectomy reversal.<h2>Vasectomy reversal recovery</h2>Full recovery after a vasectomy reversal can take one to two weeks, Pineda says, and the pain is likely to be more intense than your vasectomy recovery, but still relatively mild overall.According to Pineda, after the procedure you may experience:<ul><li>Mild scrotal discomfort </li><li>Bruising and swelling of the scrotum</li><li>Bleeding</li><li>Infection, though the risk is low - <a href="https://www.bestvasectomy.com/risks-of-vasectomy-reversal/#:~:text=If%20an%20infection%20were%20to,reversal%20is%20less%20that%203%25." target="_blank" rel="noopener">less than 3%</a> </li></ul>Patients are usually <a href="https://www.urologyhealth.org/urology-a-z/v/vasectomy-reversal" target="_blank" rel="noopener">advised</a> to wear scrotal support, like a jockstrap, for about a month afterward, avoid strenuous activity, and do not lift anything heavier than 5 to 10 pounds for about four weeks, Pineda says. Yih recommended patients not ejaculate for a full month after their surgery. Ejaculation can stress the recently reconnected tubes and increase the risk of failure from scar tissue blocking off the connection.<h2>Insider's takeaway </h2>While a vasectomy is meant to be a permanent form of contraception, it is possible to reverse it through surgery to become fertile again.
