Doctors remove live cockroach from Delhi man's small intestine, explain the insect might have entered while sleeping

Digestive problems, including ulcers in one's food pipe or <keyword id="60887" type="General" weightage="20" keywordseo="stomach" source="Orion">stomach</keyword>, could increase the risk of <keyword id="77524" type="General" weightage="100" keywordseo="parkinsons-disease" source="Orion">Parkinson's disease</keyword> by 76 per cent, according to a new study. Analysing endoscopy reports of 9,350 patients, the authors found that people having upper gastrointestinal conditions -- specifically, ulcers or other types of damage to the lining of the oesophagus, stomach, or upper part of the small <keyword id="700917" type="General" weightage="20" keywordseo="intestine" source="Orion">intestine</keyword> -- were far more likely to develop Parkinson's <keyword id="6361612" type="General" weightage="20" keywordseo="disease" source="Orion">disease</keyword> later in life. These findings, published in the Journal of the American Medical Association Network Open Researchers add to a growing body of evidence that ageing-related or neurodegenerative disease, long thought to originate in the brain, could begin in the gut. Gastrointestinal problems are known to be common in patients suffering from neurodegenerative disorders, the authors said. The researchers at the Beth Israel Deaconess Medical Center, US, said that gastrointestinal troubles experienced by patients of Parkinson's disease often appear up to two decades before symptoms such as tremors or stiffness in arms or legs, which interfere with one's movement and are usually the grounds for diagnosis. They said that digestive issues can involve constipation, drooling, difficulty in swallowing and a delayed emptying of the stomach. Constipation and difficulty in swallowing were strong risk factors related with a more than doubling of Parkinson's disease risk, the authors said. One of the possible biological mechanisms underlying these relationships between the gut and risk of Parkinson's disease could be problems in regulating dopamine, a brain chemical known to play a key role in digestion, they said. The authors also proposed that gastrointestinal conditions could trigger the building up of the protein 'alpha-synuclein', which is how Parkinson's disease presents in the brain. Future research could help understand these mechanisms better, they added.

Doctors were at a loss for what to do, so they prescribed antibiotics "like candy," she said.Over the past five years, she had taken six courses of antibiotics a year. But they didn't help. She said she needs to repopulate her gut with healthy bacteria by eating a diverse range of plant foods, but that makes her symptoms flare up.<h2 id="8a2d41da-0019-407d-9c17-cdd4e33c759c">Koepke turned to poop transplants because she felt there were no other options</h2>An FMT starts with blending a healthy donor's feces with saline solution and inserting it into the recipient's gastrointestinal tract, either through an enema, oral capsules, colonoscopy, or upper endoscopy.Experts in the documentary warned against trying poop transplants at home because there is a risk of transferring bad microbes and susceptibility to all kinds of diseases."The microbial community inside our gut can have surprising influences on different parts of our body," Jack Gilbert, a microbial ecologist at UC San Diego, told Business Insider. Stools are screened before clinical FMTs, and anything that could cause major problems, such as certain pathogens, would be detected, he said. When you do this at home, you don't get that kind of screening."With fecal microbiome transplants, there is really compelling evidence, but the science is still developing. We're still working on if it actually has benefits for wider populations and if the benefit is long-lasting," Gilbert, told the documentary.<h2 id="01134d2f-df80-410a-917a-0252392ec22b">She got acne like her brother and depression like her boyfriend after FMTs</h2>Initially, Koepke made fecal transplant pills from her brother's donated stools. She slowly started to gain weight despite not changing her diet and was able to go to the bathroom naturally for the first time in three years.However, she also developed acne, like her brother, who had a history of hormonal acne.It's possible that the bacteria in the stool can influence inflammation in the recipient's body, by affecting their metabolism and activating their immune response, Gilbert told BI. This would cause shifts in their hormonal activity, which could promote the bacteria that can cause acne on the skin. "We nearly all have this bacterium on skin, but it is often dormant," he said.Koepke decided to switch donors and, for a few months, used her boyfriend's poop for the <a target="_blank" href="https://www.businessinsider.com/poop-fecal-transplant-skin-cancer-melanoma-study-2023-7">fecal transplants</a>, who had no physical health problems but did experience depression. After she made the switch, her acne went away, but this time, she experienced depression symptoms."Over time, I realized my depression was worse than it's ever been in my life," she said. Koepke believes whatever microbes were playing a role in his depression were transplanted into her.When Koepke once again switched back to her brother's poop transplants, the depression went away within a week, she said.Gilbert said that according to his research, people with depression are missing certain bacteria from their gut. "She may have had the 'anti-depressant' bacteria in her gut, but when she swapped her microbiome with his, her anti-depressant bacteria got wiped out," he said.

There is no cure, but treatments can include drugs, changes to the diet, and surgery.Despite taking medication, Charlie Curtis' Crohn's symptoms got so bad that he was hospitalized and was going to the bathroom forty times a day, his mother, Sky Curtis, told Cassaday.Desperate for her son to get his life back, Sky Curtis contacted Thomas Borody, the director of the Centre for Digestive Diseases in Sydney, Australia, who pioneered FMT treatments in the 1980s. He suggested that she donate her healthy stools to her son.After her stool was tested for any infections or diseases, they began doing the transplants on Christmas day 2008.They did them "every day for a month, then every two days for a month, then every third day for a month. Once we got down to once a month, we did once a month for three and a half years," with newly donated stool each time, Sky Curtis told Cassaday.And it worked. "When I was actually doing the procedure," Charlie Curtis said, "I'd feel tingling inside of me. It felt healthy, it felt like it was working."Charlie Curtis was symptom free and off medication when Cassaday spoke to him in 2019.<h2>Curtis experienced his mother's menopause symptoms</h2>Charlie Curtis experienced some unexpected side effects from the FMTs: sweating, hot flashes, and mood swings, similar to what his menopausal mom was experiencing."At the time, I was going through menopause," Sky Curtis said in the documentary. "And so was he!"The donated poop does "transmit high levels of hormones," Borody told Cassaday, so it's possible that Charlie Curtis was experiencing the same symptoms as his mom because he was absorbing <a target="_blank" href="https://www.insider.com/guides/health/reproductive-health/period-poop">hormones</a> from her poop, although that has not been proven.This isn't the first time that an FMT is thought to have caused changes outside of the gut in patients. In one 2019 study involving autistic children, as well as helping gastrointestinal issues, FMTs were linked to improvements in social responsiveness and their developmental age, while multiple studies suggest that they can improve symptoms of Parkinson's Disease such as sleep disturbances. FMTs are centered around the idea that the health of bacteria living in our digestive system, known as the <a target="_blank" href="https://www.insider.com/resistant-starch-gut-healthy-recipe-2023-9">gut microbiome</a>, can in turn shape our own health.<a target="_blank" href="https://www.insider.com/fda-warns-against-poop-transplants-following-e-coli-death-2019-6">FMTs</a> are currently only approved by Health Canada and the FDA for C diff. bacterial infections, not ulcerative colitis, which can be treated using various types of medication, including immunosuppressants, and surgery.But in 2019, after Cassaday found research suggesting that FMTs have the potential to treat ulcerative colitis, she decided to take matters into her own hands.Dr. Lee Kellingray, a gut microbiota researcher at the Quadram Institute in the UK, wrote in <a target="_blank" href="https://theconversation.com/diy-faecal-transplants-dont-try-this-at-home-218083">The Conversation</a> that the DIY approach to FMTs "carries significant risks" because unlike medically supervised procedures, there is not the "strict screening" of poop donors."Unfortunately, these DIY 'success stories' may lead to more people believing that they may be cured of afflictions through repeating the process, but there is no guarantee of success and a very real threat of complications in later years," he said.<div type="image" title="Saffron Cassaday gagging after putting boop in a blender." height="1080" width="1920" msid="105452149" hostid="261" sizebytes="36580"></div><h2>The FMTs worked at first, but then Cassaday's symptoms returned</h2>First, Al Mukadam, her now-husband and then-poop-donor, did blood and stool tests to confirm he was <a target="_blank" href="https://www.insider.com/signs-you-may-not-be-healthy-2018-5">healthy</a> and disease-free.Mukadam pooped into a bucket, and Cassaday blended it with saline solution and administered it via an enema. She'd then lie for a while with her legs up, or on her knees with her butt in the air, to keep the poop from coming back out.At first, the FMTs seemed to work. By day 11 of doing them daily, Cassaday was symptom-free and off medication.But after six weeks her symptoms returned, despite continuing with the daily FMTs, so she restarted her medication. A <a target="_blank" href="https://www.insider.com/jimmy-kimmel-live-first-colonoscopy-aired-on-tv-video-2018-3">colonoscopy</a> after two months showed that although her symptoms had returned, her colon was much healthier than it was before the poop transplants. So, she continued with the FMTs, and did a total of around 200 over the course of two years.Her ulcerative colitis never fully went away, but her symptoms seemed milder, she said in "Designer $hit."<h2>Cassaday hasn't struggled with ulcerative colitis since getting pregnant</h2>Then, two months into her first pregnancy in 2020, Cassaday's colitis symptoms "completely disappeared," she said.And, three years later, they haven't come back. As Cassaday's body worked to not reject the "non-compatible tissue" of the baby, it may have also worked as an immunosuppressant for her colitis, he said.There's some evidence to support Cassaday's experience. According to the Centers for Disease Control and Prevention, the exact cause of ulcerative colitis is unknown, but it is thought to be an autoimmune condition. Research suggests pregnancy can help autoimmune conditions go into remission, although they can later return. However, other studies suggest that <a target="_blank" href="https://www.insider.com/guides/health/conditions-symptoms/do-i-have-ibd">inflammatory bowel diseases</a> such as ulcerative colitis are associated with a higher likelihood of premature births and stillbirths.Since we don't know the cause of ulcerative colitis, the research isn't clear on the relationship between pregnancy and <a target="_blank" href="https://www.insider.com/guides/health/reproductive-health/high-risk-pregnancy">autoimmune disorders</a>, and Cassaday herself acknowledges that her case is anecdotal. It's not certain that her pregnancy or the FMTs worked to "cure" her ulcerative colitis, or that they would have the same effect for other people.Insider previously reported on another one of Dr. Borody's patients, who seemingly <a target="_blank" href="https://www.insider.com/mom-donates-poop-fecal-microbiota-transplant-son-experiences-menopause-2023-11">cured his Crohn's disease</a> by doing FMTs, but also experienced his mother's menopause as he used her poop."Scientists are working hard alongside doctors to understand what is needed to be able to provide FMT for a wide range of potential treatments and getting closer every year to reaching this goal," Kellingray wrote, adding: "In the meantime, reports of DIY FMT are likely to increase as success stories are reported in the media. But the final message should always be clear that doing so is gambling with their health."
