<ul class="summary-list"><li>Auto-brewery syndrome is a rare illness where a person's body produces alcohol inside their gut, which can lead to drunkenness without consuming alcohol. </li><li>Sometimes known as gut fermentation syndrome, it's caused by an imbalance of bacteria and an overgrowth of fungus in the digestive system. </li><li>In a case study, doctors successfully treated a 47-year-old man with auto-brewery syndrome with a fecal transplant after other treatments, like a <keyword id="2291861" type="PERSON" weightage="60" keywordseo="low-carb-diet">low-carb diet</keyword> and anti-fungal medications, failed to help. </li></ul>After undergoing a course of antibiotics, a 47-year old man noticed he was feeling drunk at seemingly random intervals throughout the day — even without consuming alcohol. This rare condition, known as auto-brewery syndrome, occurs when the body produces alcohol inside the gut. Doctors tried to treat the man with a low-carb diet and antifungal medications, typically effective for this condition. But it didn't help. The patient still experienced periodic, accidental drunkenness, in one case so severe that he lost his driving license after a random police check. What finally did work was a poop transplant, according to a case study published August 18 in <a href="https://www.acpjournals.org/doi/10.7326/L20-0341">Annals of Internal Medicine.</a>Doctors were able to transfer the fecal microbiota, the bacteria present in poop, into the patient's small intestine. 34 months later, he continues to be symptom-free, according to the study. The transplant was taken from the patient's 22-year-old daughter. <h2>Auto-brewery syndrome causes yeast overgrowth to produce alcohol inside the body</h2><a href="https://www.insider.com/rare-case-of-urinary-auto-brewery-syndrome-diabetes-bladder-alcohol-2020-2">Researchers have theorized that overgrowth of fungus</a>, specifically certain types of yeast, is behind the unusual phenomenon. Those fungi feed off of carbohydrates a person consumes and produce alcohol, just as yeast can feed off grains to produce beer outside the body. In these cases, patients often become highly intoxicated from the alcohol in their digestive system, even though they haven't been drinking. In <a href="https://www.insider.com/man-drunk-on-bacteria-auto-brewery-syndrome-2019-10">one extreme example, a man was found to have four times the legal limit of alcohol in his blood due to auto-brewery syndrome.</a>Since blood sugar levels fuel the yeast-driven booze production, diabetics and people with cirrhosis can be particularly susceptible to auto-brewery syndrome. So can people who have undergone digestive tract surgery, or who have recently completed a course of antibiotics, as both can disturb naturally-occurring gut microbes and create an imbalance.That was true for this newest case study. In addition to having recently taken antibiotics, the patient also had a gastric bypass surgery years earlier.<h2>A low-carb diet is sometimes effective in treating the syndrome </h2>Typically, a low carb diet can help treat this syndrome, since fewer carbs in your system means less sugar for the yeast to convert to alcohol. An antifungal medication can also help tame the overgrown yeast. In this case study, however, nearly two months of low-carb eating and four weeks of medication were apparently unsuccessful.The study authors recommend that <keyword id="1890486" type="PERSON" weightage="60" keywordseo="fecal-transplants">fecal transplants</keyword> be considered in similar cases of auto-brewery syndrome. "He would get drunk out of the blue - on a Sunday morning after being at church, or really, just anytime," Barabara Cordell, the dean of nursing at Panola College in Carthage, Texas, <a href="http://www.npr.org/blogs/thesalt/2013/09/17/223345977/auto-brewery-syndrome-apparently-you-can-make-beer-in-your-gut?ft=1&amp;f=1007&amp;utm_source=twitterfeed&amp;utm_medium=twitter&amp;utm_campaign=nprscience" target="_blank">told NPR's The Salt</a>. "His wife was so dismayed about it that she even bought a Breathalyzer."
The condition is known as gut fermentation syndrome, or Auto-Brewery Syndrome. The man showed up at the hospital reporting dizziness and with a blood <keyword id="6359858" type="UNKNOWN" weightage="60" keywordseo="alcohol">alcohol</keyword> level of 0.37 - enough to kill a person.
That's according to the <a href="http://www.scirp.org/journal/PaperInformation.aspx?PaperID=33912" target="_blank">report from the International Journal of Clinical Medicine</a>. He was likely infected with the yeast from something he ate or drank - which could have been compounded by the fact he dabbled in home-brewing and likely had plenty of live yeast around the house.
This isn't the first time this syndrome has shown up - it's been seen a few dozen times under different names. That doesn't mean you have to fear the yeast in your home brew kit. Nor should you swallow it in hopes of making your own gut-brew.
The yeast will just slip through the intestines of healthy people without lodging in there. Each case seems to follow a round of antibiotics, which kills off the local, healthy <keyword id="6376020" type="UNKNOWN" weightage="60" keywordseo="bacteria">bacteria</keyword> leaving a space for the yeast to grow.
Now, that's a <keyword id="6359860" type="ORGANIZATION" weightage="60" keywordseo="beer">beer</keyword> gut.

In <a href="https://affiliate.insider.com?h=c11e08c822ef3c38c20fa4a4598f1eb9df406e05cb6d5061db0eed95803605a8&amp;postID=64aee531c7f3f053a27ced33&amp;site=in&amp;u=https%3A%2F%2Fwww.nature.com%2Farticles%2Fs41591-023-02453-x" target="_blank" rel="noopener">a new study</a> published in Nature Medicine on July 6, researchers transplanted the stool from one healthy donor into 20 patients with advanced melanoma who were about to undergo immunotherapy. The idea was to transfer the donor's microbiome into the patients to promote the growth of healthy bacteria in their guts and improve their cancer treatment results.While the study's primary goal was to see if this treatment is safe to try on patients, the findings were promising: 65% of patients responded to the combined treatment of fecal transplants and immunotherapy."These exciting results add to a rapidly growing list of publications suggesting that targeting the microbiome may provide a major advance in the use of immunotherapy for our patients with cancer," said Dr. Wilson H. Miller Jr., one of the researchers and a professor in the Departments of Medicine and Oncology at McGill University, in a <a href="https://www.sciencedaily.com/releases/2023/07/230707111654.htm" target="_blank" rel="noopener">press release</a> from Lawson Health Research Institute where the research was conducted.<h2>Improved gut health can make immunotherapy more effective</h2>Immunotherapy is a relatively new innovation and has grown in popularity as a<a href="https://www.insider.com/grandson-grandmother-both-diagnosed-melanoma-skin-cancer-immunotherapy-2023-4"> skin cancer treatment</a>. It works by boosting the body's immune system and helping T-cells identify and attack cancer cells. The specific immunotherapies tested in this study are called immune checkpoint inhibitors, or PD-1 inhibitors.PD-1 inhibitors can be a breakthrough treatment for some people with cancer, but for other people it has mixed or no results — <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8992532/#:~:text=Approximately%2045%25%2D60%25%20of,39%2C46%2C47%5D." target="_blank" rel="noopener">about 40% of melanoma patients</a> don't respond to the treatment.To improve this resistance to treatment, researchers in the study took a healthy donor's fecal matter, screened and prepared it in a lab, and then gave it to each patient in the form of 40 oral capsules a week before they started immunotherapy.<a href="https://www.insider.com/guides/health/diet-nutrition/gut-health">Gut health and the immune system</a> are closely connected. Dr. Bertrand Routy, oncologist and director of the Centre hospitalier de l'Université de Montréal's Microbiome Center, <a href="https://montreal.ctvnews.ca/fecal-transplants-could-be-used-to-battle-skin-cancer-montreal-study-1.6473922" target="_blank" rel="noopener">told CTV News</a> that fecal transplants could "eliminate certain bacteria that are associated with resistance to immunotherapy" as well as increase the bacteria that aid treatment.<h2>There's still some mystery as to why fecal transplants work</h2>While fecal transplants have been used to treat everything from <a href="https://www.insider.com/feeding-c-section-babies-mothers-poop-could-improve-immune-health-2020-10">C-section babies</a> to <a href="https://www.insider.com/a-poop-transplant-treatmente-auto-brewery-syndrome-gut-producing-alcohol-2020-8">auto-brewery syndrome</a>, researchers are still trying to maximize their effectiveness.Routy told CTV News that "we're beginning to understand which bacteria are associated with resistance and which bacteria are associated with the right response." He also said that the fecal compatibility between a specific donor and a patient can also play a role in the treatment's success.<h2>More research is still needed</h2>Because phase I of the experiment was deemed safe, researchers are now moving on to phase II, where they will test the effectiveness of fecal transplants in a larger group of cancer patients, according to the press release.It's still too early to know if fecal transplants will be a good treatment in the long run. A <a href="https://www.ncbi.nlm.nih.gov/pmc/articles/PMC8992532/#:~:text=Approximately%2045%25%2D60%25%20of,39%2C46%2C47%5D." target="_blank" rel="noopener">2019 study</a> found that 45-65% of melanoma patients respond to PD-1 inhibitor treatment without fecal transplants anyway. Lawson researchers also plan to study the use of fecal transplants in other cancers, such as renal cell carcinoma, pancreatic cancer, and lung cancer, the press release said, and they're also exploring how this method can impact treatments for HIV and rheumatoid arthritis.
