I couldn't do the things I wanted to do: feel comfortable approaching a girl, speak in front of the class, do drama. I'd go crimson every time.I also had excessively sweaty palms. No mother wants to see her child suffer. She was supportive based on the information we learned from the doctor and our reading.The surgeon glossed over the side effects like they were nothing to worry about. He briefly mentioned I might get some excessive sweating in other parts of my body. That was about it.By the time I got the surgery, I was 16. It involves <a target="_blank" href="https://medlineplus.gov/ency/article/007291.htm">deflating the lungs one at a time</a>, then destroying a portion of the sympathetic nerve trunk in the thoracic region, which disrupts neural messages that give the brain signals about emotional reactions, environmental changes such as temperature, and triggers including fight or flight.<h2>The side effects have lasted decades</h2>The surgery took less than an hour, and it has changed my whole life. I'm 40 now and still experiencing weird side effects — none of which were disclosed beforehand.In fairness, my hands are now mostly dry. And the facial flushing initially improved, though it wasn't completely resolved. At the time, I was devastated about that. I'd wanted to get my life back. It'd come to be the least of my worries.The lifelong side effects of ETS surgery have been brutal.I learned that, for some, cutting the sympathetic nerve reduces responses to strong emotions, such as fear or laughter. I'm now robotic in my emotional states. There's an ever-present fog; I feel disassociated. ETS has been <a target="_blank" href="https://forum.effectivealtruism.org/posts/6o7CuxJGcnckTkg49/sympathectomies-the-new-lobotomy-2">compared to a modern-day lobotomy</a> by some who've suffered similar <a target="_blank" href="https://www.youtube.com/watch?v=mL50n4-xskc">side effects</a> as me.I feel robbed of my adult emotions. But I know I'm missing something. It's often pointed out by those around me.There are other barbaric side effects, such as thermal dysregulation. I have varying body temperatures occurring at once. It's a never-ending battle. There are bed stains from how much I <a target="_blank" href="https://www.businessinsider.com/guides/home/products-that-prevent-night-sweats">sweat at night</a> — especially on my neck, back, and chest. This happens even when it's cold.I'm also left with <a target="_blank" href="https://www.businessinsider.com/guides/health/conditions-symptoms/low-blood-pressure#:~:text=Insider's%20takeaway,you%20may%20need%20medical%20attention.">low blood pressure</a>, which affects my heart rate. It has created some deeply unpleasant situations, especially in my early 20s. My resting heart rate was sitting at that of a professional athlete. I'd wake up feeling like I was going to die, with terrible palpitations. My heart would skip beats. I spent a lot of the time feeling frightened. It took awhile for that to get better.I've spoken with my primary-care doctors over the years, but they have limited knowledge of my condition and no practical ways to mitigate these side effects. I've also seen specialists such as <a target="_blank" href="https://www.businessinsider.com/cardiologist-takes-supplements-folic-acid-b12-vitamin-d-2023-10">cardiologists</a>. But I remain largely in the dark and continue to suffer.I've had to live with the decision I made ignorantly as a teenager my whole life. After going through four rounds of preterm labor, Slaman finally gave birth at 36 weeks. It was successful.Had she known the surgery was an option, Slaman believes she "probably would've been the second in the world to have it."When Slaman confronted her doctor after learning about the surgery, she said he admitted to knowing about it, but felt no obligation to tell her because it was still considered new and experimental.Despite the conversation with her doctor, Slaman believed the surgery could have saved Steven's life. She wrote a 32-page letter to De Lia, telling him she wanted to dedicate her life to working with him.In 1995, the <a target="_blank" href="https://tttsfoundation.org/">TTTS Foundation</a> launched its first website — something Slaman credits as a "huge catalyst of change" as more parents of twins learned about the surgery. There are currently about 20 centers in the US that offer the procedure.De Lia died in 2022, but Slaman — along with Matthew and her younger sons Dillon and Logan — continue to work at the TTTS Foundation. Slaman is now the president and treasurer of the foundation, and said the non-profit provides grants to families who need the treatment but can't afford to fly to one of the centers."I've helped over 30,000 women, and the grief goes so deep that helping them kind of distracts from the loss for me," she said. "I just wanted to make it better for the next one than it was for me."Slaman said it's been gratifying to see attitudes towards the surgery, and De Lia himself, change over time. In 2023, De Lia was posthumously named "<a target="_blank" href="https://journals.lww.com/greenjournal/citation/2023/05000/julian_e__de_lia,_md__one_of_the_fathers_of_fetal.6.aspx">one of the fathers of fetal surgery</a>" by the American College of Obstetricians and Gynecologists.As for her, she sees her ongoing work as fulfilling the promise she made to her sons in 1989. December 7, the day Slaman delivered Matthew and Steven, is now recognized as World TTTS Awareness Day.

This technology is one of the latest advances in minimally invasive brain and spine surgery. The 3D visualisation is guided by a scope from outside, compared to the usual 2D visualisation from inside in the endoscope. Many centres in India have 2D exoscopes, but 3D exoscope in the Postgraduate Institute of Medical Education and Research is the first in India. This is very helpful to deal with problems on the surface of the brain, spine, and orbit. Compared to a microscope, the system is lighter, occupies less operating room space, and is ergonomically better, a PGI statement said. This procedure would be of great benefit to patients with brain and spine problems, it added.

Picturing the narrow hole that swallows the occupant of the machine brought to mind the confines of a submarine or rocket."It's cool, like a spaceship," I told Matthew, as much to allay my fears as to prevent his.As a younger child, Matthew could hardly sit still at home. He'd climb trees in our backyard, clown around for a camera, and run from room to room in his Superman pajamas. Yet when he worked on a task like building a Lego spaceship, he was laser-focused. Well behaved in school and a straight-A student, he earned a spot in the gifted program.At 8, he developed facial tics, and his energy started to wane. Over the next three years, his motor tics piled on, his grades tanked, and he became clumsy, easily tired, and forgetful. We took him to myriad specialists, and none of their diagnostic hypotheses panned out. I didn't expect much of this MRI; it was simply another test to rule out something sinister.Less than an hour after I tucked in Matthew on the gurney, my husband, Mike, and I squeezed into a lab room among several technicians and a thin, white-coated man introduced himself as the radiologist. Pointing to a computer monitor, he explained that the growth — which at the time was the size and shape of a kidney bean — clung to Matthew's brain stem, a structure deep inside the brain that controls and regulates vital bodily functions, including respiration, heart rate, and blood pressure. "It's inoperable," he said, "but benign." The word "benign" kept me upright that day. Everything I knew about that word told me what he'd just said meant the tumor wasn't cancerous. In my lay world, "benign" also meant harmless. I was wrong.<h2>Learning the difference between benign and malignant</h2>Sometime in the blur following Matthew's diagnosis, the neurosurgeon on call one day told us it didn't matter whether a brain tumor was benign or malignant. With no additional explanation, his words made no sense and were too scary to consider, especially when I'd just begun to have hope. Thus, I didn't investigate the full meaning of his words until years later. When I did, I learned that even if a growth in the brain did not invade and damage healthy cells — characteristics of a malignancy — it could crowd and damage structures of the brain that enable us to breathe, think, move, and feel. For a child, whose brain is still growing and forming connections, the danger is magnified. This means that even a benign brain tumor can be deadly. For this reason, <a href="https://www.abta.org/about-brain-tumors/brain-tumor-education/" target="_blank" rel="noopener">the American Brain Tumor Association prefers the term "nonmalignant"</a> to make a distinction between the way they act.My misunderstanding of benign wasn't the only wrong assumption I'd made about Matthew's condition. The neurosurgeon told us Matthew had a pilocytic astrocytoma, a low-grade, slow-growing mass, rare in itself but <a href="https://www.hopkinsallchildrens.org/ACH-News/General-News/What-Are-the-Most-Common-Childhood-Brain-Tumors" target="_blank" rel="noopener">common among brain tumors in children</a>. Like most kids, the doctor said, Matthew would "bounce back." The more I learned, the more I thought, "If my child must have a brain tumor, this is the one to have."But the danger in Matthew's pilocytic astrocytoma lay in its location. It trapped cerebral spinal fluid in the ventricles — the four cavities in the brain — and expanded them like water balloons, a condition called <a href="https://www.mayoclinic.org/diseases-conditions/hydrocephalus/symptoms-causes/syc-20373604" target="_blank" rel="noopener">hydrocephalus</a>.I had heard of babies being born with "water on the brain," but I didn't know it could happen in older children; in fact, it can even happen in adults. Matthew's hydrocephalus was so severe that it squished his gray matter into a sliver against his skull. Every part of his brain was affected, which explained his cognitive, motor, and emotional decline over the previous three years.<h2>Figuring out the right treatment — and the aftermath</h2>While Matthew's tumor itself was inoperable, the recommended treatment for his hydrocephalus was a novel surgery — an <a href="https://www.cincinnatichildrens.org/health/e/endoscopic" target="_blank" rel="noopener">endoscopic third ventriculostomy</a>. I understood only the basics: The neurosurgeon would poke a hole in the third ventricle, and the excess fluid would drip away to be absorbed by the body. Unless the tumor grew significantly, which was not expected, chemotherapy and radiation wouldn't be necessary.After Matthew's surgery and two-night hospital stay, I believed that the nightmarish medical mystery we'd been going through since he was 8 years old was solved. It is believed that there is no reliable test available to diagnose pancreatic cancer in early stage. Diagnosis on pancreatic imaging can be done through USG, CT scan, MRI, etc. However, to treat cancer and confirm it one needs to get a biopsy done. Back pain could be the first warning sign of pancreatic cancer, study shows

In addition to risks from anesthesia and botched procedures, patients are taking a major gamble that they will share the same aesthetic ideal with their plastic surgeon.
Enter visual surgery, or "morphing" - the use of technology during the consultation process so that the patient and doctor can agree ahead of time on the surgical end results.
I visited the <a href="http://www.drsteinbrech.com/drsteinbrech.htm" target="_blank">plastic surgery office of Dr. Douglas Steinbrech</a> in New York City, whose practice has been using morphing technology for the past seven years.
<h3>What Is "Morphing"?</h3>
In short, morphing is the use of image manipulation software to give patients an idea of what they will look like once their surgery is complete.
The surgery visualization has a few upsides, Dr. Steinbrech explained. First, patients can get an idea of what they will look like after the surgery, and decide if they want to go more or less extreme than they had originally planned.
"Patients tend to be more conservative," said Patient Coordinator Bernadette Mahoney. "I think a lot of it comes from anxiety, and not wanting to take it too far. But we've actually found that when patients don't go as extreme as they want, they often regret it."
The process also helps Dr. Steinbrech determine whether a potential patient is ready for cosmetic surgery. If a patient has unrealistic expectations, it can raise a red flag. He said he might then choose to provide the patient with information on body dysmorphic disorder (an illness in which individuals are excessively concerned with body image, specifically "defects" they perceive in their own personal appearance), or simply tell them that working together wouldn't be a good fit.
But the biggest upside of all, according to Dr. Steinbrech, is that he can use the "morphed" before and after photos to make sure the results are exactly what the patient wants.
"On the day of surgery, we review these images during prep with the patient, and that has helped me nail my results as I continue to check back with the technology during surgery," he said.
The strategy has been highly effective for his practice. "A normal conversion rate [from consult to surgery] for a plastic surgery office - and this is a good number - is about 30%," Dr. Steinbrech told us. "Our conversion rate is between 80 and 85%, which is extraordinary."
What that number tells Dr. Steinbrech and his team is that when patients see their potential end result, it encourages them to proceed with the surgery.
"From a business perspective, I'm surprised more offices don't use it," he said.
<h3>I Had My Face "Morphed," And It Was Pretty Stunning</h3>

In the GIF above, Dr. Steinbrech's alterations to my face are subtle yet transformational.
After I spoke with Dr. Steinbrech and his staff about the morphing, I had to see what it was like for myself.
The first step of the process was photographing my face. I sat in a chair, and Dr. Steinbrech's assistant Kelly took pictures of my relaxed face from virtually every angle, occasionally having me feign looks of surprise, anger, and happiness.
She transferred the photographs into the system, and then I met with the doctor himself (though a real patient would next meet with a patient coordinator and Dr. Steinbrech would do the final retouching on the images at a later consultation).
I was amazed at how small his tweaks were - a little pinch here, a little pull there. But when he showed me the before and after images of my face, I was stunned.
"Patients often tell me that I'm not making enough of a change," Dr. Steinbrech told me during the faux consultation. "But then I show them the 'before' and 'after' images on the screen, and they can't believe it. They start to envision themselves as the 'after' version as we go through the process."
Even as a mock patient, I can attest that this was true. I found myself thinking the changes weren't that major, but what I didn't realize was that my internal view of my face slowly changed as I watched the doctor slim down my neck and make my jawline more square. Then I would suddenly be jolted back when I saw the 'before' versions of my images, where my cheekbones weren't quite as pronounced or my jaw as defined.
My immediate reaction to my actual, un-retouched face was shock: "That's not me; that's not what I look like."
I was already self-identifying with the "after" version of my face, which was extremely unnerving. Psychologically speaking, the high conversion rate of Dr. Steinbrech's office started to make sense - his patients may have been ready to go under the knife after watching their pictures transform because in their minds, they already looked like the "after."
My visual surgery process took roughly 45 minutes, though the average morphing consultation can take much longer, and a real patient's face would be tweaked multiple times by both the patient coordinator and the doctor.
All in all, it was fascinating to watch Dr. Steinbrech at work. It was like watching an image of myself get worked over in Photoshop, except that this man had the power to change me in real life, too.
"I always under-promise and over-deliver," Dr. Steinbrech told me at the end of the day. "I would never show a patient something on the screen that couldn't be duplicated in real life. We have bones there for a reason!"
Note: I asked Dr. Steinbrech to show me what the following procedures would look like on my face. These are not representative of the procedures he would choose to do, and should not be taken as such.

Dr. Steinbrech is well-known for this procedure. He does a little supplemental liposuction on the neck and adds Juvederm (a <a href="http://www.webmd.com/vitamins-supplements/ingredientmono-1062-HYALURONIC%20ACID.aspx?activeIngredientId=1062&amp;activeIngredientName=HYALURONIC%20ACID" target="_blank">hyaluronic acid</a>) into the jaw and chin to make it more angular. He also gave me cheek augmentations using Juvederm to provide more definition.
Recovery time: Less than 24 hours for Juvederm, roughly a week for liposuction.
Cost: $2,300-$3,800

The next procedure I asked to have virtually done was a nose job. Dr. Steinbrech gave me a "Heidi Klum" nose, which was slightly more narrow than my own.
Recovery time: Short-term recovery is roughly two weeks, but it may be several months before all swelling has subsided.

Next, he showed me what I would look like with a very minor brow lift. "You don't want to go overboard here and look too surprised," he said.

I Had My Face 'Morphed' By A Plastic Surgeon And Was Shocked By The Results

Where <keyword id="41759" type="ORGANIZATION" weightage="60" keywordseo="Nuclear">Nuclear</keyword> Failed, Oil Succeeded
 
In a continuation of our series on the state of the oil industry we look at some of the other ramifications of what we are labeling the Oil Renaissance in the US, and around the world for that matter. This phrase was first proposed regarding the potential Nuclear turnaround here in the US, where companies like NRG Energy, Toshiba and many more players all along the supply chain were positioning themselves for the Nuclear Renaissance of cheap, and abundant Nuclear energy for the next 50 years. Well, the natural disaster in Japan changed that movement in the span of a week of just untenable radioactivity readings coming out of Japan. An already uphill battle for changing public sentiment towards the dangers of nuclear energy became an impractical fight from an investment standpoint that relied upon large DOE loan guarantees to attract private investment. 
It is ironic, but all these companies spent a lot of time and effort from lobbying to developing strategic partnerships with each other, and in the end, most of that 7 year effort had to be written off by firms. It really shows how firms have to get the industry right; Oil was so much the smarter play. Higher margins, better technology, much easier safety hurdles, and even the environmental fight is much more manageable.
Not to mention the number of jobs created is far more with an Oil Renaissance as opposed to a Nuclear Renaissance, even with a complete buildup of the entire nuclear supply chain. Nuclear projects are just not scalable like oil projects are from a numbers standpoint due to the regulation, lead times for components, inspection, build times, and many more constraints. 
No DOE Loan Guarantees: The Free Market at Work 
We are going to have a Renaissance in this country, it just happened under everyone`s nose. The free market of high oil prices for the last 10 years made it happen all on its own without government subsidies, and part of the reason that things are going to get real tough for the alternative energy folks over the next 5 years as those government subsidies wind down. They will not make sense from an economic standpoint once oil prices come down considerably, and from a budgetary perspective we can no longer afford this propping up industries that cannot sustain themselves on their own merit in the free market. A 16 trillion dollar debt and climbing means the environmentalists will now be facing an uphill fight on Capitol Hill to have their cause funded by the American taxpayer. 
Technology Changes: Heart Surgery meets the Oil Patch 
The technology changes alone in the oil industry are amazing; just watch a horizontal drilling or fracking video and it is like all the advances made by the medical community for endoscopic procedures and advanced heart surgical techniques have been applied to the oil industry. And the cost is far more manageable than the medical field with all the added insurance costs, out of control bureaucracy, and government intervention all but eliminating any sense of free market principles.
Sure these constraints exist in the oil industry, but the healthcare industry is on a planet of its own and worse from a cost efficiency standpoint by a factor of at least a 100. There is not an ounce of free market in the healthcare industry!
We haven`t seen anything yet as this new technology being refined and implemented here in the US will then be fully scalable around the globe, and the amount of new projects that will come online globally with this new technology over the next ten years has yet to be priced into any market intelligence models. 
Natural Gas Industry as the Model 
The natural gas industry is much smaller than the oil industry, and because of the new technology firms were actually continuing production with $2 natural gas because of much lower overall project costs relative to the size of the gas exploitable and other derivative products made along the way enabling these projects to be profitable. 
The oil industry is much more scalable from a cost standpoint, and once these upfront costs have been committed, the size of the industry and scalability means that projects can continue and be highly profitable even with much lower oil prices. 
I previously have thought that this technology would suffer as prices drop, but I am rethinking this assumption with natural gas as my guide in a much less scalable industry. So I now believe that this technology and these projects will continue and be cost effective even with oil dropping to $45 a barrel for both Brent and WTI.
<div class="image-container float_right" style="width:275px"><div class="image"></div>slant and horizontal drilling</div>It won`t happen overnight, but under one scenario prices will just steadily trend down like natural gas prices, and before we realize it we have the equivalent of $2 natural gas prices for the oil industry. 
The China Factor: Use less <keyword id="35149" type="PERSON" weightage="60" keywordseo="commodities">Commodities</keyword> for Next Decade 
My assumption about the trajectory of oil prices also relies on the China factor that many analysts have been toying with for the last couple of years, but the IMF and others have done some nice research on and applied some hard numbers to the conceptual idea that China has overinvested for the last decade by a large degree, and most of the previous forecasts for China`s growth trajectory from an infrastructure standpoint for the next 10 years are far too optimistic. 
My conclusion is that China will use far less commodities than they did the past decade going forward for the next decade. They are coming into the constraints of large numbers where you have built for the sake of building, and you can no longer build another large new city every year because the demand just isn`t there. Basically, the easy, low hanging fruit has been eaten. Most of the new project benefits will not justify the cost based upon infrastructure constraints, logistical incongruities, and actual demand &amp; societal need for said projects.
The societal costs outweigh the societal benefits and the projects evaluated in total become a net drag on growth and GDP in the overall calculus. China can go ahead with these projects but the law of diminishing returns, means the country will pay a heavy price to do so. China will continue to grow, but they will grow in a more sophisticated way from a social perspective from within, i.e. in a metaphorical Maslow`s – Hierarchy of Needs manner, and less of a brute, infrastructure driven manner. 
Ergo, the lower utilization for commodities by China is another factor that will put downward pressure on Oil and other commodities over the next 5 to 10 years. 
More Storage Capacity Needed Globally 
Make no mistake these oil and commodity projects are going to go full stream regardless of price due to sunk costs, more efficient operations, job creation, and overall profitability.
One of the takeaways out of this analysis is that storage facilities will have to be upgraded and new ones coming online for all commodities. For example in Oil, my analysis concludes that Cushing will need to upgrade capacity to over 100 million in the next couple of years, and over 150 million by 5 years’ time.
My new analysis determines the need for even more pipelines being built out of Cushing as well. There will need to be at least 5 million barrels per day outflow from Cushing to refineries by five years’ time; can anyone say job creation opportunities here?
The next substantial upgrade besides the paltry 300,000 per/day upgrade this year will not come online until mid-2014 and only improve capacity to 850,000 barrels per/day outflow from Cushing which is not going to be enough to counter an exponential measure of domestic production coming into the Cushing energy hub by 2014. 
But I am forecasting that not only will Cushing be above 100 million in storage in three years’ time, but the US will need capacity to store over 600 million barrels by four years’ time, and China who is building storage currently, will need to meet their own need for storage due to a massive oversupply in their country.
China was building storage initially for strategic purposes, but my analysis concludes that because of an oversupply issue similar to copper today in China, they are going to need this additional storage for excess supply issues.
Therefore, if you’re in the storage facility business, times will be good for the next five years, plenty of business for these firms. As I think storage facilities will have to be built all around the world from Iraq, Saudi Arabia, Africa, and the Scandinavian countries. 
A New Price Model for Oil
So how low can prices go? Let`s just say that the Renaissance in oil is going to be good for the global economy, just back in 2003 gasoline prices were $1.60 a gallon in the US and oil was trading around $30 a barrel. 
It is not unreasonable to think if the Oil Renaissance takes the path that it is capable of that Oil globally trades all the way down to the $45 area. 
What Price do the Saudi`s Really Need? Need &amp; Want Confused
And those that think that OPEC would need $75 to keep up production, remember that OPEC still kept pumping oil only four years ago with $33 oil in 2008. Furthermore, OPEC countries still need the overall revenue not the price per say.
Accordingly, you could very easily have a scenario where prices go lower and they pump more, violate reduction quotas because they all want the revenue net of volume and price, not just less volume but slightly higher prices.
I think the world will be surprised how the talking your book rhetoric of “we need $75 oil to justify production” is replaced with the actual, “we need the money and our real cost is so much lower than you could ever imagine” reality on the ground.
This is their one asset in these countries, some revenue stream is better than no revenue stream, and with global production picking up OPEC `s relevance, power, and influence on prices is diminishing by the day. Great OPEC you can reduce production, your global competitors will love that, less competition for them.
The only problem is that these countries need the money, every country needs the money these days, and that`s the market place you take what you can get on the market! The market goes in cycles, just as the housing market re-priced itself, so will the oil market! 
The ironic point here is that often the lower prices go, the more oil that is produced trying to make up in volume for the lower price to get as much revenue as possible. 
$45 Oil &amp; $2 Gasoline: Consumers Love this New Era 
In conclusion, we are entering a new Renaissance in the oil market, not just in the US, but globally as well. New technology, slower growth in the emerging markets over the next decade, and an era where a decade of high prices will finally bear some fruit with market dynamics working as their supposed to leading to more supply, and an eventual reduction in prices. 
Expect this new era to manifest itself in giving the entire world a tax break, and small businesses and consumers worldwide will have more disposable income, and sectors such as retail, entertainment, transportation, and global travel will benefit as a result of this sea change in the oil industry.
I could even envision the manufacturing industry in the US getting a large piggyback effect as the US will have some of the cheapest energy costs of anywhere in the world for starting a business with an abundance of natural gas, oil and petroleum products for the next decade at a low and stable price. 
How does $45 a barrel oil and $2 a gallon gas sound? Something the peak oil folks thought was an outright impossibility just 5 years ago. 
But a lot of things can change real fast, once technology gets involved, impossible things become possible. Just look at smartphones versus 4 years ago, and the fact that I can deposit a check into my checking account via my smartphone, actually surf the net, get usable navigation directions, and watch Netflix movies on my smartphone.
The world history of scientific innovation being applied to markets is remarkable to say the least looking back, it’s now time for the oil market to have its second renaissance. Expect $45 oil in the future of this renaissance.
