More than half of the population of Bihar – at 58% – does not have access to even a midwife let alone doctors who are usually available at sub-centers. Yet, a large number of rural areas lack proper healthcare infrastructure – and in particular, the states of Bihar, Jharkhand, Andhra Pradesh, Uttar Pradesh, and West Bengal – which face an acute shortage of PHCs. A PHC or a primary healthcare center (PHC) is manned by at least one doctor covering a population of 20,000 people, has at least four beds, and is preventive and curative while a sub-center offers very basic services like immunization. Fewer PHCs means more ‘distance’ in healthcare access and extra burden on existing centres. Indian rural healthcare centres are so few and so burdened that people are avoiding them

As India reopened after two dreadful years of Covid-19, the country saw a re-emergence of infectious diseases and a new variant of the hand-foot-and-mouth disease, also known as <keyword id="13885432" type="General" weightage="100" keywordseo="tomato-flu" source="Orion">tomato flu</keyword>, created quite a scare across many states. <keyword id="6384473" type="General" weightage="80" keywordseo="tomato" source="Orion">Tomato</keyword> flu is a rare viral infection that could be a new variant of the viral hand-foot-and-mouth disease, a common infectious disease targeting mostly children aged one to five years. Initially, <keyword id="28504" type="General" weightage="50" keywordseo="kerala" source="Orion">Kerala</keyword>, <keyword id="20154" type="General" weightage="50" keywordseo="tamil-nadu" source="Orion">Tamil Nadu</keyword> and Odisha reported tomato flu cases. Tomato flu was first identified in the Kollam district of Kerala, on May 6, 2022. The Kerala Health Department took precautionary measures to monitor the spread of the viral infection and prevent its spread in other parts of India. In September, Assam reported over 100 cases of tomato flu, sounding an alarm for the state health department. The maximum number of cases were reported from two schools in Dibrugarh district. The <keyword id="6357713" type="General" weightage="50" keywordseo="uttar-pradesh" source="Orion">Uttar Pradesh</keyword> and Tamil Nadu governments had also issued advisories on tomato flu. Though the tomato flu disease is not seen as life-threatening, healthcare experts warned that any further spread of the disease may hit schools again after the Covid-19 pandemic. Health officials also said that the chances of adults contracting the disease are feeble. Bhawuk Dhir, dermatologist at Delhi's Ram Manohar Lohia Hospital, said that this is a clear sign of heading towards an era of viral diseases. "Clearly, we are heading towards an era of viral diseases with the rampage of Covid-19, Monkeypox and now hand-foot-and-mouth disease -- as was predicted by various researchers in the past," Dhir had told IANS. "Tomato flu is caused by Coxsackie virus A16 (a non-polio enterovirus), which is highly contagious and spreads through secretions from the nose, throat, fluid from the blisters and fecal-oral route," Dhir added. In most cases, it is a mild self-limiting viral disease and requires supportive care for recovery. A few may develop complications such as meningitis and disseminated infection. The Union Health Ministry also issued guidelines on Tomato flu, underlining that its treatment is similar to other viral infections like isolation, rest, plenty of fluids and hot water sponge for relief from irritation and rashes. Supportive therapy of paracetamol for fever and body ache and other symptomatic treatments are required. "Tomato flu is a self-limiting infectious disease as the signs and symptoms resolve after a few days," the ministry guidelines said. No antiviral drugs or vaccines are available for the treatment or prevention of Tomato Flu. "The term tomato fever was used because of the red-coloured blisters which resemble a tomato. A similar outbreak was reported in Kerala in 2007," according to Dhir. Although not many new cases are being reported now, the best way to prevent this disease is maintaining proper hygiene and sanitisation of the surrounding environment as well as preventing the infected child from sharing toys, clothes, food, or other items with other non-infected children.

Even as climate models have indicated an increasing heatwave this summer for <keyword id="3658" type="General" weightage="80" keywordseo="india" source="Orion">India</keyword>, people suffering from asthma, a respiratory disease, may be at significant risk, said experts here on World Asthma Day. World Asthma Day is an annual event observed on the first Tuesday of May, aimed at raising awareness about asthma, a chronic respiratory disease affecting millions worldwide. According to the <keyword id="43588" type="General" weightage="80" keywordseo="world-health-organization" source="Orion">World Health Organization</keyword>, approximately 339 million people suffer from asthma globally, and the number is expected to rise to 400 million by 2025. "Heatwaves can be a challenging time for individuals with asthma, as extreme heat can worsen symptoms and trigger an asthma attack," Dr Nikhil Modi, Senior Consultant, Respiratory and Critical Care Medicine, Indraprastha Apollo Hospitals, told IANS. "The high temperatures and increased humidity can cause airways to become inflamed and constricted, making it difficult to breathe. This can lead to shortness of breath, wheezing, coughing, and chest tightness," he added. Moreover, hot weather also increases the levels of air pollutants, such as ozone, which can further aggravate asthma symptoms. The global climate crisis is fuelling heat waves to become more frequent, intense and lethal in India. In 2022, India experienced its hottest April in 122 years and its hottest March on record. And it experienced extreme weather on 242 out of 273 days between January and October 2022, revealed a recent study by Cambridge University. The study also showed that about 90 per cent of India is in the "extremely cautious" or "danger" zone from heat wave impact and almost all of Delhi is particularly vulnerable to severe heat wave impacts. Meanwhile, the India Meteorological Department (IMD), last week, predicted above-average temperatures and heat waves until the end of May in the country. It said for states like Bihar, Jharkhand, Odisha, Gangetic West Bengal, East Uttar Pradesh, coastal <keyword id="37102" type="General" weightage="80" keywordseo="andhra-pradesh" source="Orion">Andhra Pradesh</keyword>, Chhattisgarh, Madhya Pradesh, Telangana, and coastal Gujarat, heatwave days "will be more than usual". To combat the effects of heatwaves on asthma, it is essential to take precautionary measures to minimise exposure to triggers. Health experts suggested people with asthma to stay indoors during the hottest parts of the day, typically from 11 a.m. to 3 p.m., and avoid strenuous outdoor activities. "To stay protected, people with asthma should avoid exposure to extreme heat and humidity, stay hydrated, and keep your rescue inhaler with you at all times. It seems that India’s slowing <keyword id="35050" type="CITY" weightage="20" keywordseo="gdp">GDP</keyword> growth in 2017-18, in the wake of demonetisation, an overhaul of the tax regime and a decline in manufacturing and investment, has had an adverse impact on the financial strength of its states. According to <a href="http://www.careratings.com/upload/NewsFiles/SplAnalysis/Study%20of%20State%20Budgets%20for%20FY19.pdf">a report by CARE Ratings</a>, a credit ratings agency, 14 out of 24 states recorded a decline in their net financial position, as per latest estimates, in the year ended March 2018. Of the 14 states that recorded a revenue surplus, which occurs when revenue receipts are higher than revenue expenditure, eight saw a lower surplus than the year before, including <keyword id="6357713" type="COMPANY" weightage="20" keywordseo="Uttar-Pradesh">Uttar Pradesh</keyword> (₹185.2 billion), Bihar (₹14.5 billion) and Odisha (₹88.7 billion). However, a revenue surplus isn’t an obvious indicator of financial strength. Spending beyond their means Another cause for concern is that more states are spending beyond their means. The number of states with a <keyword id="6365485" type="DEFINITION" weightage="20" keywordseo="Fiscal-Deficit">fiscal deficit</keyword>, the difference between total revenue (excluding borrowings) and total expenditure, that is larger than 3% of GDSP (a state’s gross domestic product) has increased from 10 in 2016-17 to 14 in 2017-18. The 14th Finance Commission recommends a maximum limit of 3% on state fiscal deficits. In fact, 5 states actually had a deficit ratio higher than 3.5%. Bihar stood out as the state with the worst ratio, at 7.5%, partly owing to <a href="http://www.prsindia.org/administrator/uploads/general/1519878666_State%20Budget%20Analysis%20-%20Bihar%202018.pdf">higher spending on pensions, education and rural development. </a> India’s states are increasing the amount they spend on asset creation, in lieu of future growth and development. Around 14 out of 24 states reported a higher capital outlay, or the money spent on infrastructure creation and fixed assets, as a percentage of total expenditure. Goa had the highest allocation, at 27% of total expenditure, followed by Jammu &amp; Kashmir and Bihar. It bears mentioning that Uttar Pradesh, Maharashtra and Punjab all extended generous loan waivers last year. However, as shown below, it seems that only Maharashtra can afford the waivers. Maharashtra and Uttar Pradesh have the highest <keyword id="44647" type="ORGANIZATION" weightage="20" keywordseo="debt">debt</keyword> levels Maharashtra, India’s richest state, had the highest level of outstanding debt with ₹4.07 trillion, closely followed by Uttar Pradesh, one of India’s poorest states, with a debt burden of ₹4.06 trillion. This is roughly equivalent to the cost of 306,000 Lamborghini Gallardos. However, given its high GDSP, Maharashtra has a debt-to-GDSP ratio of only 16.3%, well below the Finance Commission’s maximum permissible limit of 25%, while Uttar Pradesh has a debt ratio of 29.5%. The reports show that 8 out of 19 states had a debt-to-GDSP ratio over 25%. Punjab and Jammu &amp; Kashmir had worryingly high debt ratios, at 41% and 46.4% respectively. Along with West Bengal and Haryana, Punjab spends more than 20% of its revenue receipts on interest payments alone, which results in lower development spending. Only four states - Karnataka, Telangana, Odisha and Chattisgarh - proved to be efficient at managing their debt obligations. In addition to having a debt-to-GDSP ratio of less than 25%, these states also allocated less than 10% of their revenue receipts to interest payments. An improvement in 2019? In light of their declining financial strength, India’s states will focus on fiscal consolidation and cost control in the current fiscal year. Only 7 states are budgeted to have a revenue deficit in 2018-19, out of which 4 expect to reduce their revenue deficit. West Bengal, Assam and Andhra Pradesh, all of which recorded a revenue deficit last year, expect to record a surplus or a neutral balance in 2018-19. Meanwhile, all the 14 states that recorded a revenue surplus last year, expect to do so in 2018-19 as well, with 9, including Gujarat, Uttar Pradesh, Chhattisgarh and Bihar, planning to increase their surplus. These states will continue to count on the generosity of the central government for grants-in-aid and tax revenues. According to PRS Legislative Research, a think tank, <a href="http://www.prsindia.org/uploads/media/Analytical%20Report/State%20of%20State%20Finances%202018.pdf">states derive 46% of their revenue</a> from the central government’s coffers. Grants account for 20% of state funding while the states’ share of central taxes account for 26%. Given their dependence on the central government, it isn’t hard to see why so many voters in state elections favour the party that is in power at the centre.

There was no advisory on that day (UK travel advisory was released on 18 March) that stated that I needed to quarantine myself. I showed no ill health so did not quarantine myself." The singer said she travelled to Lucknow to meet her family on March 11 and claimed there was "no screening setup for domestic flights". On March 14 and 15, Kanika said she attended a friend's lunch and dinner and clarified "there was no party hosted by me and I was in absolute normal health." However, on March 17 and 18, she developed symptoms and said she got herself tested on March 19. "On March 20 when informed that my test was positive, I chose to go to the hospital. I was discharged after there negative tests and have since been at home for 21 days. "I want to especially thank the doctors and nurses who have so graciously taken care of me during a very emotionally testing time. I hope that all can deal with this matter with honesty and sensitivity," she added.  PM CARES donations from sportsmen From the sporting arena, Indian cricketer Suresh Raina is donating ₹52 lakh but only ₹31 lakh is going towards the PM-CARES fund.  The ayurvedic consumer goods company Patanjali, founded by Baba Ramdev, has given ₹25 crore to the PM CARES Fund and set up five quarantine centers for those who have testing positive for Coronavirus. Indian tech giant Infosys, will be donating ₹100 crore through the Infosys Foundation, the company's arm for philanthropic efforts. Branches of government doing their part National Highways Authority of India (NHAI) officials are putting up one day's salary towards PM CARES. Coal India Limited (CIL) has pledged ₹220 crore and NLC India has promised ₹25 crore. All of Indian Railway's employees have pledged one day's salary to the newly-formed Prime Minister’s Citizen Assistance and Relief in Emergency Situations (PM-CARES) Fund, according to union minister Piyush Goyal. Collectively, the contribution will amount to ₹151 crore from Indian Railways Ministry.
